چکیده

The use of percutaneous mechanical circulatory support (MCS) devices, including Impella and Intra-aortic balloon pump (IABP), in patients with cardiogenic shock (CS) has increased recent times. We aimed to evaluate the impact choice MCS device on healthcare resource utilization. queried National Inpatient Sample registry between October 2016 December 2018 identify adults admitted for ACS related CS received coronary intervention (PCI). study population was segregated into IABP groups using ICD 10 diagnosis codes. Primary endpoint high utilization (HRU), while secondary outcomes included peri procedural complications. Propensity scoring modelling used matched pair patients, adjusting severity index, comorbidities. During period, 439,610 using, or account CS. Median age (years) were similar (64.1 vs. 65.1, P=0.08). Gender distribution like female majorities both groups, (71.9% 67.9%, P=0.05). had a higher comorbidity burden. 1:1 score matching identified 2,620 comparable index patients. HRU, surrogate length stay (LOS) >7 non-home disposition at discharge, compared (94.2% 87.3%, P<0.001). in-hospital mortality (42.8% vs 26.9%, also developed more periprocedural complications, vascular injury (4.6% 1.9%, P=0.004), acute kidney (61.5% 51.9%, P<0.001), requiring dialysis (5.5% 2.9%, P=0.01) when Among undergoing PCI receiving those demonstrated greater utilization, LOS days, discharge IABP. Further investigation is warranted elucidate factors associated these findings.

Is Intra-Aortic Balloon Pump Counterpulsation Sufficient to Treat Patients in Cardiogenic Shock, Undergoing Primary Percutaneous Coronary Intervention

BACKGROUND A high number of patients do not survive primary percutaneous coronary intervention (PCI) complicated by cardiogenic shock (CS), even when assisted with intra-aortic balloon pump (IABP) counterpulsation. There is no accepted consensus on who may most benefit from IABP counterpulsation, although previous retrospective studies have reported predictors of survival for patients undergoin...

Percutaneous Mechanical Circulatory Support Versus Intra-Aortic Balloon Pump in Cardiogenic Shock After Acute Myocardial Infarction.

BACKGROUND Despite advances in treatment, mortality in acute myocardial infarction (AMI) complicated by cardiogenic shock (CS) remains high. Short-term mechanical circulatory support devices acutely improve hemodynamic conditions. OBJECTIVES The aim of this study was to determine whether a new percutaneous mechanical circulatory support (pMCS) device (Impella CP, Abiomed, Danvers, Massachuset...

A prospective, randomized clinical trial of hemodynamic support with Impella 2.5 versus intra-aortic balloon pump in patients undergoing high-risk percutaneous coronary intervention: the PROTECT II study.

BACKGROUND Although coronary artery bypass grafting is generally preferred in symptomatic patients with severe, complex multivessel, or left main disease, some patients present with clinical features that make coronary artery bypass grafting clinically unattractive. Percutaneous coronary intervention with hemodynamic support may be feasible for these patients. Currently, there is no systematic ...
